Your hospital has completed its community health needs assessment (CHNA) and you have an implementation plan, but is it current? Is it measurable? Is it collecting dust on a shelf or making a difference in your community?

Join us on August 23 to learn more about the importance and practical applications of CHNAs and how to collaborate with community-based organizations. Not-for-profit hospitals are required to conduct a CHNA once every three years and develop an implementation plan to meet community health needs. While not every hospital is required to conduct a CHNA, assessing the existing health needs within your community is a best practice benefitting both your hospital and the community you serve. 

CMS payment programs are beginning to require health equity plans, and this session will also cover how information included in your CHNA can also be used in your health equity plan.

Continuous evaluation and improvement are essential components when implementing the CHNA implementation plan and health equity plan. Participants will review examples and elements of CHNA implementation plans and learn the importance of aligning the plan with hospital strategic priorities and quality initiatives. We will also spotlight resources currently available from the Mississippi State Department of Health to assist in your community health improvement projects.

Learning objectives:
Compare and contrast the health priorities identified by rural hospital CHNAs and state-level rural health priorities
Identify best practices and resources to strengthen current and future CHNA implementation plans and health equity plans
Identify health improvement strategies that can be implemented in rural communities
Identify how to integrate community, clinical and quality data for assessment
Identify community health collaborative models that maximize community benefit dollars and increase equity in underserved populations
Assist participants in the shift from planning in the CHNA to doing in the implementation plan with collaborative community partners
